Home Assessment

The Home Assessment is completed by our Assistive Technology Professional (ATP) through review of medical records, interview with the resident and/or family, and an onsite evaluation of the home. A report will be provided based on the findings, which includes specific descriptions, photos, measurements, and recommendations regarding the functionality and accessibility of the resident within their home environment. Home Assessments are utilized for hospital discharge planning, catastrophic injuries, and home environment/accessibility clarification. Home Assessments can also be beneficial in avoiding unnecessary inpatient or long term care facility placement, shorten hospital or inpatient length of stays, confirm appropriateness of medical equipment ordered, and provide home safety tips to prevent re-injury or new injury. The Home Assessment will offer recommendations for medically necessary and appropriate equipment or modifications including access ramps/threshold ramps, entry and doorway widening, kitchen modifications, bathroom modifications, furniture placement modifications, and mobility and transfer devices/aides.
