  • Home Assessment

    The Home Assessment is completed through review of medical records, interview with the resident and/or family, and an onsite evaluation of the home. A report will be provided based on the findings, which includes specific descriptions, photos, measurements, and recommendations regarding the functionality and accessibility of the resident within their home environment, as well as ability to enter and exit the home safely. Home Assessments are utilized for hospital discharge planning, catastrophic injuries, and home environment/accessibility clarification. Home Assessments can also be beneficial in avoiding unnecessary inpatient or long term care facility placement, shorten hospital or inpatient length of stays, confirm appropriateness of medical equipment ordered, and provide home safety tips to prevent re-injury or new injury. The Home Assessment will offer recommendations for medically necessary and appropriate equipment or modifications including access ramps/threshold ramps, entry and doorway widening, kitchen modifications, bathroom modifications, furniture placement modifications, and mobility and transfer devices/aides. They may be presented for review with the physician for determination of medically necessary orders and to facilitate appropriate discharge or treatment plan.

    Ergonomic Assessment

    The Ergonomic Assessment is completed by our Certified Ergonomics Assessment Specialist (CEAS) through a review of the employer’s job description, interview with the employee and the employer, and an onsite evaluation of the employee in the primary location where they perform their job duties. A report will be provided based on the findings, which includes specific descriptions, photos, measurements, and recommendations. Ergonomic Assessments are utilized for identifying and mitigating workplace risk factors (i.e. repetitive activities), improving workplace efficiencies and comfort, maximizing safety, increasing productivity, reducing employee downtime, and preventing injury or re-injury. The Ergonomic Assessment will offer recommendations for workplace modifications including assistive technology solutions, seating, furniture placement, work surface layout, work methods/procedures, ergonomic work practices, and human elements/positioning. The assessment report may be presented to the treating physician for evaluation of appropriate work restrictions following work place modifications.

    Job Analysis

    The Job Site Analysis is completed through a review of the employer’s job description, interview with the employer, and an onsite evaluation of the job duties being performed. A report will be provided based on the findings, which includes specific descriptions with photos of the essential functions, equipment used, environmental conditions, and physical demands of the job. Job Analysis is utilized for obtaining clarification regarding job duties when a formal job description with physical demand assessment is unavailable, and for addressing any job accommodation issues whether temporary or permanent. The report may be presented to the treating physician for assessment of physical capabilities and for return-to-work purposes, or clarification of causation.
